Classification of ISSUE-57 change proposals

I've put the ISSUE-57 proposals into the following categories, based
on the way I analyze the issue:

1. Always-content  - the representation can be assumed to be content.
(3 proposals)
2. Content opt-out  - the representation can be assumed to be content
unless indicated otherwise. (2)
3. Content opt-in    - the representation cannot be assumed to be
content absent other information. (5)
4. Other (1)

I've tried to cast a wide net and include proposals that have come up
even if they don't use the change proposal template or aren't fully
baked.
