I think the basic problem is that user style sheets challenge a couple
of major tenets of modern marketing:

1) every document you produce should re-inforce the branding of your
company;

2) the words are not as important as the emotional impact of the 
document.

As a result, authors ignore the possibility that their styling may be
ignored, so even minor tweaking, like overriding author font sizes,
will break many, possibly more than half of, sites quite badly.  Also
some vendors are really producing browsers as a platform for the
generators of commercial content, rather than as a tool for users,
so they give little priority to user control.
