[CSS21] Replaced elements and :before/:after

The current draft of CSS2.1 is not clear on what the interaction of 
:before/:after and replaced elements is.  This should be specified (and 
I thought it had been, but I don't see anything on the subject at 
http://www.w3.org/TR/CSS21/generate.html#before-after-content).
