Rules as to position of table display types within tables?

Are there any rules as to where captions, etc., can appear in markup (as
in HTML) - what about t-l: fixed  - should the ua have to wait for a
caption, or can it assume that after 1/2/350 elements that there isn't
going to be one (similarly for other types; e.g., footers, etc.).
