Updated data model specification document

I have updated the data model specification draft based on last week's
discussion.  As before, the document can be found at

http://opencreds.org/specs/source/claims-data-model/


The major changes since last week are:

- added note that terminology is undergoing changes

- noted that identity id may be long-lived but need not be

- added signed JWT example to JSON section

- added Abstract, Status of this Document

- removed unneeded 'credentials in identity' appendix

- changed 'credential' to temporary name of 'TBD credential'

Known items still to do:
- Add introduction
- Fix/update links and references
- Update JSON-LD examples to be either real or closer to real
- Change "TBD Credentials" to real name once we have it
