Regarding Failures for 2.4.6 (http://www.w3.org/TR/UNDERSTANDING-WCAG20/navigation-mechanisms-descriptive.html) I'd like to propose a failure for this: The use of numerous, irrelevant headings. For Example: <h3><a href="example.html">Some Passage of Text</a></h3> <h3><a href="another_example.html">Another Passage of Text</a></h3> By applying headings incorrectly, or for passages of text which do not delineate a section, users of assistive technology may be stripped of this useful navigation technique.
