Hi,

  I am getting various interpertations of attributeFromDefault from various
sources so...

  If the value is set to "unqualified" does that mean that:

  1)  Attributes are NOT REQUIRED to specify a namespace (but could) or
  2)  Attributes must not specify a namespace
