[mathml-core] status of old CG mathml-core draft links

Brian flagged in the call that the old link to the mathml-core draft was 
broken

As the owner of the mathml-core git repository was changed from 
mathml-refresh to w3c github automatically add server redirects for 
github.com links

So old links to the repository, issues and pull requests such as

https://www.github.com/mathml-refresh/mathml-core

https://github.com/mathml-refresh/mathml-core/pull/35

automatically redirect to

https://www.github.com/w3c/mathml-core

https://github.com/w3c/mathml-core/pull/35

*However* github pages sites are not automatically redirected.

so

https://mathml-refresh.github.io/mathml-core/

was a 404 error.

To address this I have just added a mathml-core/index.html to the 
mathml-refresh.github.io repository that does a html meta element 
refresh to redirect.

So for browsers at least

https://mathml-refresh.github.io/mathml-core/

should redirect to

https://w3c.github.io/mathml-core/
