4. 5.1 - nest heading properly - two issues:
 1.  we need to explicitly say that use of H1 conveys structure 
 2.  and distinguish between improper nesting  and skipping levels (one more
serious than other).  
 - No H3 before H2. 
 - H2 -> (H3) -> H4 -> H3
