Initial Pass at a Survey

Hello Everyone,

I made an attempt to convert some sample portions of the developing ELREQ
into a survey format.  To be clear, it is not a full survey document, I
have just selected a few sections and tried rephrasing them in survey form
to see how it would look and discover any issues that might emerge.  My
main goal has been to keep it very simple and clear to understand.  A few
changes to enhance clarity will work their way back into the spec
document.  The results can be downloaded here:

Feel free to add to it, I will as I have time. A final version will need
translation.
