EME: ISSUE boxes in the spec

In addition to addressing the V1 GitHub issues, I believe we need to remove
all the red ISSUE boxes from the spec before PR. Any that might result in
substantive changes need to be addressed by June 9th.

EME currently has nine ISSUE boxes. Some have corresponding GitHub issues
while others do not. I'll file issues and/or create PRs as appropriate.
