Should the label for 4 be “Very important” if 2 is "Somewhat important”?

The instructions mention “5 stars” and “1 star” but the form doesn’t use stars as labels. The instructions could also explicitly state that if the respondent doesn't understand a criterion, they can skip it.

The question “Are there no trademark or IP issues?” could be rewritten as a statement: “There are no trademark or IP issues”.

When you fill in the form and click on “Done”, you just get the same form again. A confirmation message would be nice.

Hi,
thank you for preparing this. I'm not sure this is the right format
for a survey. There are over 30 criteria (I stopped counting at 30),
each rated on a 1-5 scale, and you can select 10 key criteria. This
setup tends to dilute the results. For example, should I choose the
ten criteria I value most and rate them all as "Somewhat important -
Essential" (4-5 scores), or should I only assign a perfect 5 to a few
while rating the rest 5 lowest?

What data do you expect to get from this survey, and what's the overall goal?
