Thank you for reminding those, sorry for I didn't realized this step,

Yes, no new review is needed.

Because the errata is used to record the errors after publication, so I 
remove the previous changes record to summary document [3], but during 
LTO announcement period the errata remained the summary record during 
review process, I did those change after LTO announcement [4]. The 
errata page [2] will adopt after publication, sorry for confusion.

Yes, all previous comments have been updated to latest CAT version. For 
validation issues, LTO and I will fix them before publication, thanks 
again for Richard's comment.
