Note in DTD that ref & nullable mutually exclusive?

A tad late but,

I don't see a note in the structures DTD for the definition of "element"
that if there is a ref attribute then there may not be a nullable
attribute (i.e., ref and nullable are mutually exclusive).  /Roger
