There has been some communication failure on my part, leading to
confusion about this issue in the Primer.  The new PWD [1] makes clear 

 a) <complexType> without <simpleContent> or <complexContent> daughter 
is equivalent to
    <restriction base="anyType>...</restriction>

 b) No content model particle (<all>, <choice>, <group>, <sequence>)
    == empty content
