correction to 'Document Type Declaration'

I believe the line which says 'Document Type Definition' just above 
the productions 28 and 29 should say 'Document Type Declaration'.

By the way, it is unclear to me why the productions 1, 22, 28, 29, 
30, 31, 39, 43, 45, 46, 47, 48, 49, 50, 61, 62, 63, 64, 78, 79 are not
defined with regular expressions, as indicated by lower case of the 
initial letter of a production label (same as a symbol in EBNF).
