Transfer

In the VXML 2.0 doc, in section, 2.3.7. TRANSFER, the various possible transfer outcomes are listed.


There doesn't appear to be away to distinguish between the cases:

(1) Caller disconnected before transfer was completed.
(2) Caller disconnected after transfer was completed.

The documentation states that 
near_end_disconnect means "The call completed and was terminated by the caller.".  This seems to be the correct value for (2), but what about (1)?
