FWIW, German sometimes also dropped the diacritic but always by decomposing uppercase umlauts, i.e. ‹A›/‹O›/‹U› plus ‹e› or ‹E›, with the latter on the line. I think some years ago someone proposed an extension to ‘text-transform’ which could handle such cases. It would make ‘::first-letter’ even more of a nightmare than it already is, though, so it’s probably better to rely on authors typing two separate letters.
