According to the standard, in a  Qualified Name Sub-module one is 
supposed to set the entity XHTML.xmlns.extra.attrib to 
%MODULE.xmlns.extra.attrib; and perhaps use the DTD driver to change 
this value (which is necessary if one wants to combine many modules). 
However,
_before_ redefining this entity, one needs to instantiate  Qualifed 
Name Sub-modules to know the MODULE1.xlmns.extra.attrib, ..., 
MODULEn.xmlns.extra.attrib entities. But this will assign a default 
value to XHTML.xmlns.extra.attrib and makes impossible to change it.
Do I miss something?
