Hi I'm not going to pretend to be able to answer all your questions but I think http://www.xml.com/pub/a/98/10/guide0.html may do a better job than I could anyway. Also I'm not sure on what the policy on recommending books on this list is, but there are some books mentioned as related reading in and around that article that I have found very helpful in beginning to understand the topics you are asking about.
