Yes, understood.

We work with screen magnifiers, on screen keyboards and voice input 
software as well.

Realistically, screen readers tend to be the pickiest and most difficult 
to drive changes into. They're also expensive for users to update. I think 
most of the effort goes into screen readers.

there were a number of modified keyboard type devices,  Dvorak has
been around since the 30s and was usable in DOS
