All these references and links need to be updated. I'm not sure this document is still needed. Some things I liked about it - such as being able to review all the report sections at once. Actually I think this is still very useful and relevant. For example, I was recently in a meeting where they were discussing a common reporting format for all public websites in Europe so that they can be monitored. It occurred to me that only very few people know how to go directly to the "view report" step of the report tool to get a template report (and even then, things like "executive summary" are missing from that view). Many - especially non-technical people - might not start fiddling with tools to see if it can generate a report template, not matter how clear we make it in the instructions for the tool... Anyway, I think this could be a very very simple thing (and hopefully I didn't jinx it now) - basically copy and paste the "view report" stuff from the report tool, add any missing bits, and update the references to WCAG 2.0.
