The use of the complex search should be left as a last resort. You should keep the search simple by not giving options. But it you must the select box would be the only accessible way. The previous example left out the label for the "Search." This show that you can not associate more that one label to a component. The input type of radio would require more than one label, but it is impossible to do. The select box would be the only choice, you can associate "Search" with select, and "for" with the text box.
