[w3c/push-api] Rewrite .subscribe() to fix various bugs (PR #350)

The following are not testable/observable, but spec bugs nonetheless:  

  * Fix `PushSubscriptionOptionsInit` usage.
  * Do less work in parallel.
  * Handle `userVisibleOnly` member.
  * Resolve promises using networking tasks source.
