Extended & edited questionnaire, and marked-up document from today's TF meeting

Hi gang,

We handled many of the items on our second survey - trying to close on 
all last edits before asking WCAG WG to approve it for public review 
publication - but still have a few left.  And there were a couple items 
from that survey that we partially answered.

So, as directed by the TF, please review & answer the UPDATED Agreeing 
our "Final draft before requesting publication as a Working Group Note" 
<https://www.w3.org/2002/09/wbs/55145/20130618/> survey.  The items we 
closed on should all be clearly marked.  The items we didn't get to are 
left as-is.  And the two items that we worked on but still have some 
questions left are also so marked.

Please pay particular attention to question #13, where I tried to 
capture thoughts on ways of de-emphasizing (or de-emphasising as they 
say across the pond) that came up among the handful of folks who 
remained on the call after the TF meeting ended.  The 4th suggestion in 
that survey question assumes it might be possible to place an HTML 
button next to each "Intent from <SC>" line that would do the 
expansion/collapsing, something we haven't yet verified with Michael.  
So you may want to hold off on that vote...


Finally, attached please see the attached document "WCAG2ICT editors 
draft 6June13 - with PK edits and GV edits - TF meeting edits.doc". It 
should contain all of my proposed (but not yet discussed or approved) 
edits to chapter 5 (the old chapter 5 - comments by guideline and SC), 
chapter 7 glossary entries, and the appendicies. It also contains my 
live-edits of everything we discussed & approved today (on the overall 
structure, abstract & status, introduction, key terms, closed 
functionality, text/command-line/terminal, and conformance sections).
