Synchronizing with Richard's cleanup of R2RML, I have cleaned up the
Direct Mapping section of [1]. I haven't changed the math, apart from
clarifying the treatment of NULLs, but I have cleaned up the rendering
in HTML.

I believe this section is ready for Last Call.
