We state in 3.1.2 "At evaluation time, the value of a variable reference is the
value of the expression to which the relevant variable is bound". However, we
have a number of constructs that associate a value with a variable by means
other than evaluating an expression: examples are the positional variable in a
for clause, the various window variables, and the variables in group-by.
