Second paragraph in section 18.2.1 reads:
"The default namespace (as declared by xmlns) may be designated as an extension
namespace by including #default in the list of namespace prefixes."
It doesn't mention what happens if the parent element has no default namespace.
A new error should be added to disallow this.
