With HTML::Template cache => 1 and disabled error message loading:

  Concurrency Level:      50
  Time taken for tests: 817.78125 seconds
  Complete requests:      2500
  Failed requests:        0
  Write errors:           0
  Total transferred:      133567500 bytes
  HTML transferred:       132850000 bytes
  Requests per second: 3.06 [#/sec] (mean)
  Time per request: 16341.563 [ms] (mean)
  Time per request: 326.831 [ms] (mean...)
  Transfer rate:          159.64 [Kbytes/sec] received

I.e. 1.70 => 3.06 [#/sec], that's indeed a bit better. I could not get
Apache::DProf to work, without -DONE_PROCESS it only writes the header
for the profile data and with -DONE_PROCESS Apache simply exits when
loading a mod_perl script.

As soon as someone does whatever is needed...
