provenance model document

Dear all,

Paolo and I have made a large number of edits to the provenance model 
document.
Its latest version is available at [1].
Doing so, we tried to identify the consensus that was emerging from 
email discussions,
and formulated proposals to address raised issues.  After doing so, we 
decided to close
many issues pending review.

We invite you to review the latest version of the document. If you find 
that issues were incorrectly closed,
by all  means, reopen them.  Given that the document has significantly 
changed, it may be
more suitable to create, more precise issues.

We have to recognize that some sections, such as derivation, are still 
work in progress.
Other sections are entirely novel.
