Revised draft HTML5 license survey

In response to the comments made by WG members the Chairs have prepared a revised draft HTML5 license survey.  The revised draft survey includes the following changes:

a) adds a fourth question about the CC0 license proposed by Mozilla,
b) changes the introduction to correctly reflect the additional fourth question,
c) changes all questions to use radio buttons,
d) ensures that each question has a Rationale text box that can be used by a respondent to explain their position on a question, and
e) changes the title of the first three questions to identify them as "W3C license option x"

Note that the Chairs have NOT added a question about the MIT license since we have not heard from anyone in the WG directly advocating for the MIT license.
