I've added a first draft of AVT support in the XForms 2.0 RNC schema as
asked in this action:

1. I didn't verify every attribute agains the specification
2. I didn't put AVT on @ref or @nodeset because it wasn't clear to me
which we were doing and which we weren't
3. I asked a question about adding header/@name and header/@value and
have added them in to the RNC for now.
Note that header/@value is slightly different from
header/value/@value because you can have multiple header/value[] elements.
