Nick, In http://www.w3.org/MarkUp/Forms/wiki/XForms_2.0#The_header_Element I don't see any notations on the header/@combine. Also, should we add header/@name and header/@value back-formed as constants from header/name/@value and header/value/@value and mark those as AVT? Leigh.
