Processing old DID spec issues

Just a heads up that I started processing our oldest DID spec issues.

This is an attempt to cull the number of open issues on the DID spec
down as we try to finalize the technical/implementation parts of the
spec. Around 11 issues have been closed and this is just a heads up to
the group that this is happening so that those that don't watch the
repository know what's going on.

You can see all the most recently closed issues, as well as the
reasoning behind closing them, below:

Almost every issue that was closed is something that the group has come
to consensus on but where we forgot to update the issue tracker.
