The Survey for publishing Making Content Usable

It will be included in the AGWG agenda we send out next Thursday as well
but we are sending it out early to allow extra time for review due to the
document's size.

This survey will be discussed at the 23 March AGWG meeting. If consensus is
reached at that meeting, a CFC will then follow.
