Registration Questionnaire

Hi, folks–

I put together this questionnaire [1] which I propose as an alternative 
form of registration, rather than requiring a position statement.

Please let me know what you think. If you have suggestions, let me know 
right away.

If we agree that this is a good approach, we can push this out by the 
end of the day today, and extend the deadline to next Friday.


As a reminder, please help promote this workshop in your circles, 
especially if you think this form is a better way to get interesting 
attendees rather than the position statement.
