Dynamic dispatch: p:apply extension?

  I remember we talked in Lyon about the possibility of a new p:apply
facility to run a step the type of which is computed dynamically:

  Norm, I can see you added the note "Please consider adding the above
to EXProc’s proposed steps!"  Just curious: has it been any advancement
on this one since then?
