Also, how did a talented group of experts missed something as fundamental this even after investing two years? I was one of those 'experts' yet I dropped the ball as well. I am genuinely interested in knowing what can be done to reduce, if not remove, this sort of mistakes. I hope we are not going to file it under 'nobody is perfect' and forget about it.
