It appears to me that the third example is incorrect.  Note that as printed,
one "
" of attribute "a" is converted to #xD in the case where attribute
"a" is CDATA.

Please accept my apologies if this has already been reported.  I am not
connected right now, so I do not have access to the errata list, and since
it confused the dickens out of me, I wanted to make sure that I reported it.
