The first example in appendix D "Exansion of Entity and Character References" is

	<!ENTITY example "<p>An ampersand (&#38;#38;) may be escaped

It is then stated that the "example" replacement text is

	<p>An ampersand (&#38;) may be escaped
	numerically (&#38;#38;) or with a general entity
	(&amp;amp;).</p>

However,

	(&amp;amp;).</p>" >

would resolve to

	(&amp;).</p>" >
