Section 4.4.4 enumerates forbidden things, which constitute fatal errors. I infer from the wording that this list is meant to be complete. But comparison with the table in section 4.4 makes me think that it is not. I suggest adding the following bullet item to section 4.4.4: the appearance of the name of a parsed entity as an attribute value. (or, "as a token in the value of an attribute of declared type ENTITY or ENTITIES") This in turn makes me question an entry in the table. Should the cell at the intersection of "Parameter" and "Occurs as Attribute Value" not say "Forbidden" instead of "Not recognized"? Thank you.
