revised 'loose' HTTP binding

Plese find attached a revision of my candidate for a SOAP binding to
HTTP; it incorporates some recent discussion, and can be thought of
as leaning towards the 'loose' side of things. Comments appreciated.

As Chris stated regarding his, this draft reflects my thoughts and
not necessarily those of the WG as a whole. This document is for
