The RTF output of Apache FOP is rather limited. Properly mapping XSL-FO to RTF is quite difficult to begin with (100% can never be achieved). Since you're talking about a TOC, you probably have trouble with fo:leader which isn't supported for RTF output. At the moment, you're out of luck here. I don't know if XMLMind's FO Converter is better at converting your document to RTF in this case. Worth a try: a free trial is available.
