Suggestion for XML Schema 1.1: I am frustrated by the inability to extend all-groups. I would very much like to see a model in which all-groups can be extended either via type derivation or via inclusion of named all-groups in all-group definitions, with the restriction that no all-group may contain any subgroups that are not themselves all-groups. This would, it seems, keep the whole thing fairly tractable (a simple substitution semantics like that for attribute groups should suffice), and would allow all-groups to be built up modularly, which would be a big help. This would entail no change to the interpretation of currently valid schemas, hence no backward incompatibilities, and it strikes me as very compatible with the current scope and flavor of XML Schema.
