QName interpretation is underspecified in the Datatypes REC, and this will be corrected. The issues list contains a proposed resolution [1] to this problem which has been ratified by the WG, and I'm responsible for drafting the erratum which will settle the matter. Here's the relevant section from the above reference: This should be clarified in the REC -- the intention is that unprefixed names are qualified iff there is a default namespace declaration in scope, i.e. as per element names, not attribute names, in XML 1.0 plus Namespaces.
