The following sections have no content, and appear to be a
recapitulation of the list immediately above them in 2.16 [1].  Can
these redundant and empty sections simply be removed?  I suspect they
are a hangover from XML 1.1 support.

  2.16.1 NCName Type
  2.16.2 anyURI Type
  2.16.3 QName Type
  2.16.4 boolean Type
