How does CCXML handle VoiceXML 2.1 Consultation transfer?

CCXML 1.0 W3C Working Draft 29 June 2005 provides an appendix section to
describe the details of integration with VoiceXML 2.0. This section captures
the details of handling blind and bridge <transfer> operations.
 
VoiceXML 2.1 enhances the <transfer> element by adding consultation
transfer. I would like to get some pointers about handling consultation
transfer in CCXML. In general, is there any working document/tutorial that
explains CCXML 1.0 - VoiceXML 2.1 integration details?
