Is it possible the Validator is somehow finding a different config 
file - for example the one in htdocs/config, rather than /etc/w3c?

You might consider using Validator-SAC, which is a stand-alone Mac 
version of the W3C Validator. I created Validator-SAC because Apple's 
instructions are complicated and out-of-date:

Setting up Validator-SAC as a web service, just involves copying one 
file to /etc/apache2/other. See the advanced instructions:

Validator-SAC can also be a useful reference for building the 
Validator on Mac OS X, because it comes with source code and build 
scripts.
