I thought that you don't want to address the "ruby with emphasis dots" 
issue is CSS3 (which makes sense since this is indeed an edge case), but 
then the only reasonable way to handle this is to leave it unspecified. 
IIRC, CSS2.1 leaves several situations unspecified for forward 
compatibility.

Now that you specify it. Chances are that it will be a feature at risk :)

Notice that on the right the emphasis dots are not aligned, while on the 
left they are. So this is indeed complicated.
