Most people would do something like:

  <TITLE>foo</TITLE>
  <H1>bar</H1>
  <P>....</P>
  <H1>baz</H1>
  <P>....</P>
  <H1>qux</H1>
  <P>....</P>

apply a transform, and get:

  <A HREF="/TITLE"><H1>foo</H1></A>
  <A HREF="/H1=1"><H2>bar</H2></A>
  <A HREF="/H1=2"><H2>baz</H2></A>
  <A HREF="/H1=3"><H2>qux</H2></A>

as a TOC, but then again, I might just be pulling your leg ;-)
